MEMBERSHIP CATEGORIES & ENROLLMENT FORM   In Celebration of City Club's 25th Anniversary, the Club is offering a very      Special one-time Enrollment Fee of $25!          Contact Monica DeLaTorre, Membership Director, at 817.878.4088 or    email her at mdelatorre@cityclubfw.com  for enrollment information.     A variety of memberships are available, with one-time Enrollment Fees ranging from $150 to $200.     Monthly dues within each membership category vary for Individuals, Couples, Families, and Juniors.    These categories range from $45 to $165 per month.     FULL MEMBERSHIP    Full membership provides fitness privileges, as well as dining privileges for business and social purposes.     Many Group classes in Fitness and Mind/Body Studios are included.     SOCIAL MEMBERSHIP    Social Membership provides dining privileges for business or social purposes.     JUNIOR FULL AND SOCIAL MEMBERSHIP    Junior membership provides applicable privileges to members 35 years of age and younger.    NON-RESIDENT MEMBERSHIP    Non-resident membership provides the privileges of Social membership to members    who office and reside outside of Tarrant County.     Click here for the Membership Enrollment Form    Please fax or e-mail your completed Membership Enrollment Form to:    Monica DeLaTorre    Membership Director    817.878.4088    817.878.4034 (fax)   mdelatorre@cityclubfw.com
